General description (provided by company)

Main purpose

moveUP provides a platform to be able to monitor and adapt the treatment patients: during, before and/or after an intervention.

Depending on the chosen treatment, the patient can be monitored during the full treatment journey by a single or multidisciplinary health care team.

The personalised treatment service is offered through a web interface or App on tablet or smartphone and provides individualised treatment advice depending on the chosen pathway (e.g. medication, coaching, information, activity, exercises, ...)

As a patient, you have the possibility to interact with your care team through the App (depending on the chosen pathway)

The individualisation of the treatment is based on the medical profile and progress of parameters throughout the treatment. For this, the health care team can use the feedback through daily (or other frequency) reported parameters (eg. symptoms and vital parameters) as well as optional sensors which capture relevant activity. When needed a picture or video can be requested to assess the progress of functional performance or skin/wound related parameters.

The treatment and start of the care plan can be started based on the intervention timing (e.g. before surgery, or from diagnosis)

Target audience

moveUP.care is designed to serve multiple user groups within the healthcare ecosystem within different care pathways: patients and health care professionals.

Patients: The platform offers digital support and engagement tools tailored for individuals across various medical domains, including orthopedics (arthroplasty and conservative treatment), cardiology (surgery and heart failure), oncology (haematology), abdominal surgery, obesity, respiratory (COPD, asthma), and neurology. Patients benefit from personalized care plans, educational resources, and tools to monitor their health, all aimed at enhancing therapy adherence and overall outcomes.

Healthcare Professionals (HCPs) and Hospitals: moveUP.care provides advanced digital tools to maximize efficiency for healthcare providers. Features such as predefined care pathways, integrated communication functions (tele expertise and tele monitoring), and comprehensive patient overviews assist in streamlining decision-making and patient monitoring and management. moveUP.care facilitates ERAS trajectories (enhanced recovery after surgery).
This support enables HCPs to deliver personalized and precise care.
